The following is a listing and usage description of current SBE program forms:

MSDGC Small Business Supplier Diversity Directory: Vendors by Company Name

Vendors registered with MSDGC. Vendors are certified by public entities whose certification criteria does not exceed MSDGC certification criteria. For the purpose of MSDGC’s SBE Program, all vendors registered are considered small businesses. It is your responsibility to verify the SBE firm has a current certification.  Our system provides a grace period to update the file once a certification expires.  The SBE firm is not removed from the system until the grace period has expired.  Lastly, beginning April 1, 2011 the cert. start indicates the date the firm became active in the MSDGC SBE Program.

Professional Services Monthly Report Monthly Utilization Report for Professional Services contracts/contractors.
MSDGC Small Business Registration Form This form should be submitted by small businesses that would like to be included in MSDGC’s SBE Program.  Please refer to the MSD SBE Registration Highlights for more information.
SBE Goal Compliance Plan This packet contains the SBE Goal Compliance Plan Cover Page, Subcontractor Utilization Plan, Waiver Request Form and Affidavit, and SBE Subcontractors Good Faith Efforts Summary Sheets.
MSDGC Subcontractor Utilization Plan for Construction Projects (Statement of Intent to Utilize Firms) This form is used by the Prime contractor to identify the firms the subcontractor(s) that will work on the project.
MSDGC Subcontractor Substitution Form This form is used by Prime contractors when a subcontractor listed on the Subcontractor Utilization Plan (Statement Of Intent To Utilize Firms) is being replaced.
MSDGC Adding New Subcontractor This form is used by Prime contractors when new subcontractors are being added and no subcontractor substitution occurs.
SBE Language for RFPs SBE Language to be used in Project-Defined RFQ/RFP.
MSDGC Consultant\SubConsultant Utilization Plan for RFQ & RFP This form is to be used by Consultants\SubConsultants to submit Utilization Plan for RFQ & RFP.

Local Hiring Questions Includes questions that were provided to MSDGC regarding the City‟s Local Hiring law that existed prior to the August 1, 2012, ORDINANCE, (EMERGENCY) submitted by Councilmember Winburn from John P. Curp, City Solicitor on July 24, 2012, SUSPENDING temporarily the local hiring requirements of Chapter 318 of the Cincinnati Municipal Code until November 1, 2012, which brief delay will allow the director and staff of the Metropolitan Sewer District of Greater Cincinnati to have additional time to adopt policies and procedures to implement the local hiring requirements.
Local Hiring Guidelines (Rev. 05082013) The Greater Cincinnati Water Works (GCWW), Metropolitan Sewer District of Greater Cincinnati (MSDGC), and Stormwater Management Utility (SMU) have implemented City of Cincinnati Ordinance 131-2012, "Local Hiring Requirements," (Chapter 318 in the Cincinnati Municipal Code for its construction projects). This program is administered for these agencies by MSDGC's Office of Workforce and Business Development.
Local Hiring Guidelines for Water Pollution Control Loan Fund Projects (WPCLF) 05082013 Used for construction projects that receive Water Pollution Control Loan Fund (WPCLF) dollars. The Greater Cincinnati Water Works (GCWW), Metropolitan Sewer District of Greater Cincinnati (MSDGC), and Stormwater Management Utility (SMU) have implemented City of Cincinnati Ordinance 131-2012, "Local Hiring Requirements," (Chapter 318 in the Cincinnati Municipal Code for its construction projects). This program is administered for these agencies by MSDGC's Office of Workforce and Business Development.
